Online Submission for Healthcare Recipient (Patient) Registration
Notes for Online Submission
  • For person aged below 16 or a person aged 16 or above but incapable of giving consent, the application should be submitted by a Substitute Decision Maker (SDM)
  • For person aged 16 or above and is capable of giving consent but cannot submit application in person, he/she may authorise a representative to submit application on his / her behalf
  • When making online application, applicants can give sharing consent to up to three participating healthcare providers (except HA and DH)* at the same time.
*Note: After joining eHRSS, you are taken to have given sharing consent to HA and DH.
  • Upon successful submission, authenticate identity to activate eHealth record
    - Activation is an important privacy protection measure which authenticates applicant's identity to start sharing patient's electronic health record in the eHRSS. Electronic health record within the eHRSS sharable scope would be shared to the system around 30 minutes to 1 hour after record activation. Please provide the identity document(s) [as the one(s) used when making this application] for identity authentication to proceed with record activation when the patient next visits a registered private healthcare provider providing healthcare to the patient, or at any eHR Registration Centres in Hospital Authority (HA) or Department of Health (DH), or eHR Registration Office.

    (a) If you are aged 16 or above,  please bring:

    (b) If you are the SDM,  you may activate the eHealth record on behalf of the patient, please bring:
    • Your identity document
    • Copy of patient's identity document
    (c) If you authorise a person to activate your eHealth record on your behalf,  the authorised person should bring:
    • Copy of your identity document
    • Authorised Person's identity document

  • Managing sharing consent to healthcare providers upon successful record activation
    - When the eHealth record is activated, patient's registration in eHRSS will be effective and you will receive a notification of your successful registration with an Access Key via the selected communication means. Patient and his/her SDM(if applicable) can use the access key or identity document to manage sharing consents given to healthcare providers (except HA and DH)
